Patients that are candidates for surgical procedures are ones that need more correction than just a liquid facelift per se. These are people that might have a lot of extra skin or extra fat under the chin and then something has to be done to remove that. We also have to keep in mind and consider the fact that with ageing, we loose volume in our face and the fact of tightening the skin is not always the esthetic or cosmetic ideal, a lot of times we have to add volume such as the cheek area. We do that by fat transfers, which I have written papers about in the early 1980s of how to properly take the patient’s own fat to add that volume is going away. Of course, our last thing that we want to do is to create a windswept blown-back look, which the patients really do not like and neither do I, and that is because some of the physicians or clinicians miss the need for extra volume in the face.
